Gatepath – Bilingual Help Me Grow Coordinator

The Help Me Grow (HMG) Coordinator will be an integral part of the launch team for HMG in San Mateo County. HMG is a national model that builds collaboration across service sectors, including health care, early care and education, and family support. In San Mateo County,  HMG is a grant-funded program that will offer families opportunities to ensure the optimal health of their children by supporting early identification of developmental concerns and providing referral(s) to link them to customized community resources.  

The HMG Coordinator will be the primary resource for families and staff for questions regarding child development.  They will have responsibility for taking calls from families and providers through the HMG Call Center, a designated phone line to help families and providers access   services appropriate for their needs.  In addition to answering questions about child development, they will be responsible for linking families and children to existing resources in San Mateo County and helping then navigate the system of care.  They will also perform family and provider outreach.

Responsibilities

  • Respond to calls, emails, texts, referrals and warm handoffs coming into the Call Center

  • Answer parent & provider questions about child development and respond to their inquiries for information

  • Ensure families and children are connected to services they need by

    • providing care coordination for families

    • researching  appropriate resources online, in our resource database and through personal outreach to providers

    • providing education and information on child development, behavior management and services

    • collaborating with providers

  • Provide information and expertise on child development to other HMG staff, as needed

  • Perform family and provider outreach to promote the HMG system and Call Center

  • Develop in-depth knowledge of the network of services for children and families in San Mateo County

  • Collect and enter family data into STAR database system and run regular data reports

  • Collect and enter updated provider information into the resource database
